Beginning your application
Before beginning the forms below please read the guidance for voluntary accreditation materials. Prior to making an application, an organisation should submit the first two forms from table 1 below:
Applications are made to the Australian Competition and Consumer Commission, in its role as the Digital ID Regulator. The available application forms are listed in the table 1 below.

Organisation and Authorised Officer registration form - used to register an organisation with the Regulator, and to provide contact information for the Authorised Officer.

Service type and Contact Officer registration form - used by an organisation to register a service type with the Regulator, enabling the service type to apply to be accredited.
Submitting forms to the regulator
The application forms are PDFs, allowing entities to download them and provide the required information. The completed application form should be emailed to digitalIDregulator@accc.gov.au, with all required supporting evidence attached.
When emailing a completed application form to the Digital ID Regulator, the application should be sent by the entity’s Authorised Officer, and clearly identify the organisation or service that the application relates to in the covering email. The entity can also reattach a submitted Organisation and Authorised Officer registration form or Service Type and Contact Officer registration form to identify the organisation or service that the application relates to.
